Scenario 5: notification-dispatch-svc — RCA Refined

A "memory leak" that a human reviewer correctly refines into an unbounded in-memory cache.

The service is restarting repeatedly on OOM kills. The AI correctly spots a memory leak — a reviewer refines the finding: it's an unbounded in-memory subscriber-preference cache added in last week's release.

Steps to test

  1. Open INC0000005.
  2. Steps 4–7: heap_used_bytes climbs steadily with no drop after GC.
  3. Steps 10–11: melt-anomaly hypothesis at 0.74 confidence.
  4. Step 12: reviewer marks it refined, replacing it with the corrected hypothesis at 0.9 confidence.
Outcome: both the AI's original hypothesis and the human's correction are visible side by side.
